Licensed Practical Nurse

JOB SUMMARY

Morris Heights Health Center (MHHC) is seeking Licensed Practical Nurses (LPN) to provide nursing care to assigned patients under the direct supervision of the Registered Nurse (RN) or Provider in meeting the needs of the patient. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills, is skillful in mentoring and teaching and participates on committees or projects.

JOB RESPONSIBILITIES

• Perform patient interview and intake; prepare patient for triage by RN.
• Obtain full set of vital signs and recognize and report results out of normal range.
• Collect specimens (urine, throat swabs, etc.).
• Perform diagnostic point of care testing (blood glucose fingerstick, etc.).
• Document care, including phone notes in the EMR.
• Follow up on abnormal labs and radiology results.
• Administer immunizations and medications according to acceptable standards for safety and infection control.
• Participate in patient and family education with documentation of teaching and patient response.
• Obtain prior approval for medical insurance and prescription refills.
• Maintain current American Heart Association basic life support (BLS) status.
• Actively participates in patient emergency events, including code blue.
• Maintain knowledge of current trends and topics in professional practice through participation in continuing education programs.
• Maintain patient care equipment and report any malfunction to the Charge Nurse, Site Director and head of purchasing.
• Contribute to excellent patient satisfaction by maintaining professional communication with patients, family members and colleagues, and providing safe and quality care.
• Other job duties as required.

JOB REQUIREMENTS

Graduate of an accredited school for Licensed Practical Nursing; Minimum of one (1) year of clinical experience, preferably in an ambulatory care setting; Current Basic Life Support (BLS) Certification required; Must be flexible, open to change and willing to participate in new initiatives in the department.

HIRING RATE

$27.88 - $31.79 per hour (based on experience)
